Community Health Worker- $17-$18/hour
Pro Action’s Kids On Track Program is looking for a part-time Community Health Worker to visit area healthcare practices and hospitals to greet new parents to encourage their participation in Kids On Track Partnership Services, accept and manage primarily WIC program referrals and pre-screen primarily for WIC eligibility and contact families that consent to the program outreach to support a diverse community.
The Health Worker will promote the program to patients in health care settings as well as from health care referrals. Other duties include managing and tracking referrals for WIC and supporting the WIC program with general reception duties. The Health Worker will be collaborating with a variety of people with diverse populations
A High School Diploma or equivalent with 3-5 years’ experience in a related field of work is required for this position. The ideal candidate will be proficient in data entry, be organized and able to manage multiple projects and priorities simultaneously. The ideal candidate must have a driver’s license, meeting agency standards and consistent, reliable transportation that will allow regular travel in and around the service area.
